Elizabeth Hernandez-Chowdhari, MD

Primary Care Physician, Internal Medicine

Dr. Hernandez is a board-certified Internal Medicine physician with fellowship training in Geriatric Medicine from Yale School of Medicine. She partners closely with older adults to support healthy aging through compassionate, personalized care centered on each patient’s goals. Dr. Hernandez earned her undergraduate degree at the University of Miami, her medical degree from Florida International University, and completed her residency and geriatrics fellowship at Yale; she is fluent in English and Spanish and enjoys family time, the beach, fishing, travel, and reading.
